This case study is based on an actual client engagement. To protect confidentiality, all names and dashboards have been completely anonymized.

Empeek’s elderly care portfolio covered fall detection and incident response — systems that activate after a crisis. 

Missing was the preventive half. 

Unlike remote patient monitoring, the platform centers on behavioral engagement rather than physiological tracking, helping organizations intervene before prolonged isolation develops.

The evidence base is settled: 

The buyer is the organization responsible for the population, not the individual senior. One system had to serve three constituencies: participants with low tech literacy, families needing visibility without intrusion, community managers needing signals they can act on.

Challenges

Accessibility Under Real Conditions

Reduced vision, limited motor precision, minimal smartphone experience. Standard mobile UX — multiple navigation tabs, dense hierarchy, small tap targets — fails this population. Every core action had to be completed in one or two taps.

Detection Without Clinical Data

Isolation risk had to surface weeks before any clinical indicator would. That meant building a composite behavioral signal — the Engagement Score — sensitive enough to catch genuine decline without flooding community managers with false positives.

 

Intervention Without Overreach

Push notifications for wellness checks were rejected outright: unexpected alerts confuse and distress this demographic. Mood check-ins live in-app only, trading completion rate for data quality and lower user anxiety.

Process

Kindred Connect tested across several rounds with actual user groups runs as a mobile app with role-based views plus a separate admin console.

The participant view opens on a daily mood check-in, then weekly goals and a filtered activity feed: physical (chair yoga, walking buddies), social (gardening club, video coffee rooms), cognitive (chess, smartphone basics), and intergenerational mentorship.

Each activity card states intensity, duration, and physical requirements — “comfortable chair, water nearby, quiet space.”

Accessibility is structural, not a settings toggle: large type, high contrast, and reduced motion are set at onboarding and stay adjustable.

Process

The admin console runs a detection-to-action loop. Sixteen-plus days without activity, or consistently declining mood, generates a prioritized queue with ready actions — call, message, volunteer assignment — each tracked to resolution.

Family Circle gives relatives a 30-day mood trend, attendance, goal completion, and upcoming activities, without staff involvement.

Results

Measurement Window & Validation

Figures reflect a structured operational cycle across all six communities, with Engagement Score trends compared at 30-, 60-, and 90-day intervals. Detection sensitivity and false-positive rates are tracked as part of ongoing algorithm validation. All percentages are derived from platform event logs.

Every Risk Flag Itemizes

Every risk flag itemizes its own inputs: 16 days without activity (+32%), mood down 3.2 points (+22%), four consecutive missed events (+18%), 21 days without a conversation (+14%) — each paired with a recommended action.

Five metrics run in the console:

  • Engagement Score — participation, mood check-ins, and goal completion, compared at 30/60/90 days
  • Return-to-activity rate — participants resuming within 30 days of a dormant period
  • Intervention success rate — share becoming active after an outreach action
  • Recovery rate — movement from High or Medium Risk to Low Risk post-intervention
  • Program effectiveness — Engagement Score change, repeat participation, and new connections per program

 

Cohort Analytics Segment

Cohort analytics segment by age group and living situation, directing limited volunteer and staff capacity toward the demographics where retention drops. The Executive Dashboard packages the same figures for funder reporting.

Program-level Data

Program-level data is where the score becomes actionable. Across ten active programs — 1,125 enrolled participants, 84% average attendance across those programs, 446 friendships formed — Walking Groups rank first on composite effectiveness, with the largest engagement lift at +22 points and the highest friendship count. 

Volunteering delivers +24 points for participants who join as helpers rather than recipients. Learning Sessions record the lowest retention at 68%; the platform flags it and recommends testing shorter formats. A program that isn’t working surfaces as plainly as one that is.

Is this a medical device?

No. The app tracks engagement and mood, not clinical vitals, and does not diagnose, treat, or monitor a medical condition — it is built to sit within FDA general wellness guidance.

How does risk detection work without wearables?

From behavioral signals: participation frequency, mood trends, goal completion. The Engagement Score is a trend signal, not a fixed threshold — sustained decline raises the flag, so ordinary day-to-day variation does not.

Does this replace case management or clinical oversight?

No. It surfaces who needs attention, offers a ready next action — call, event invitation, volunteer visit, family check-in — and logs the outcome.

Can older adults with low tech literacy use it?

 Yes. One large daily activity card, one-tap join, high-contrast type, and accessibility defaults set at onboarding.

How does family visibility work without compromising privacy?

Family Circle is scoped to trend data — enough for a relative to know when to check in, not enough to monitor every move. Families can call or message from the same screen.
